To join the Utah Doula Association please fill out the membership form below and click "submit". After you submit your form, you'll be taken to a page with instructions on how you can pay for your membership. Memberships are not valid until paid.

Annual membership to the UDA is $25. A combined annual membership to both the UDA and DONA is available for $50 (this membership runs from April 1, 2010 to April 1, 2011). Payments made after 3/21/10 will be subject to a $10 late fee.
